Coaches Background Check

In order to participate as a coach or assistant coach on any CGAA team we are required to collect the following information in order to perform a background check. This background check will access a number of databases in order to provide the information required to approve you, protect the association, conform to our insurance requirements, conform to the City of Cottage Grove guidelines and protect the players.

Please be as complete as possible in providing the requested information. Your actual name must be used - nicknames or abbreviated versions are not acceptable for the purpose intended. There must be some information in each field to proceed with registration. If there is no information to provide insert "none" in the field.

Please review the crimes listed below that would be cause for disqualification from participating in any CGAA activities.

Background Check Crimes

If you have been convicted of any of the following crimes, you will be disqualified from participating in any CGAA activities:

  • Murder
  • Kidnapping
  • Manslaughter
  • Any assault against a minor
  • Felony Level Assault
  • Criminal Sexual Assault
  • Arson
  • Prostitution related crime

AND/OR

Any of the following Child Abuse Crimes committed against a minor victim, constituting a violation of Minnesota Statues Sections:

609.185 Murder in the First Degree
609.221 Assault in the First Degree
609.222 Assault in the Second Degree
609.223 Assault in the Third Degree
609.224 Assault in the Fourth Degree
609.322 Solicitation, inducement and promotion of prostitution
609.323 Receiving profit derived from prostitution
609.324 Other prohibited acts of prostitution
609.342 Criminal Sexual Conduct in the First Degree
609.343 Criminal Sexual Conduct in the Second Degree
609.344 Criminal Sexual Conduct in the Third Degree
609.345 Criminal Sexual Conduct in the Fourth Degree
609.352 Solicitation of children to engage in sexual conduct
609.377 Malicious punishment of a child
609.378 Neglect or endangerment of a child
152.021, subd. 1,(4) Controlled Substance crime in the First Degree
152.022, subd. 1,(5) Controlled Substance crime in the Second Degree
152.022, subd. 1,(6) Controlled Substance crime in the Second Degree
152.023, subd. 1,(3) Controlled Substance crime in the Third Degree
152.023, subd. 1,(4) Controlled Substance crime in the Third Degree
152.023, subd. 2,(6) Controlled Substance crime in the Third Degree
152.023, subd. 2,(7) Controlled Substance crime in the Third Degree
152.024, subd. 1,(2) Controlled Substance crime in the Fourth Degree
152.024, subd. 1,(3) Controlled Substance crime in the Fourth Degree
152.024, subd. 1,(4) Controlled Substance crime in the Fourth Degree

In addition to the above crimes which require disqualification, CGAA policy states that a person may be disqualified and prohibited from serving as an employee, independent contractor, or volunteer of CGAA and/or its organizations if:

  1. The person has been convicted (including crimes the record of which has been expunged and pleas of "no contest") of any crime of violence against minors, or any crime which indicates the person may pose a risk to the safety and well being of players under his/her direction of stewardship, including, but not limited to all felonies not described above, domestic assault, indecent exposure, and multiple convicts for a misdemeanor, operating a motor vehicle or watercraft while under the influence of alcohol or controlled substances within the past ten (10) years; OR
  2. It is determined that he/she has been adjudged liable for civil penalties or damages involving sexual or physical abuse of children, is subject to any court order involving sexual abuse of a minor, including but not limited to domestic order for protection, has had his/her parental rights terminated for reason involving sexual or physical abuse of children; OR
  3. CGAA or any of its affiliates comes into possession of verified information that he/she has a history with another organization (as a volunteer, employee or otherwise) of complaints of physical or sexual abuse of minors, resigned or been terminated or been asked to resign from a position, whether paid or unpaid, due to complaint(s) of sexual or physical abuse.

NOTICE

As the subject of a Child Protection Act background check, your rights include:

  • To be informed that CGAA or organizations associated with CGAA will request this check for becoming or continuing as an employee or volunteer and to determine whether you have been convicted or any of the specified crimes;
  • To be informed of the BCA's response and obtain a copy of the report from CGAA or organizations associated with CGAA;
  • To obtain from the BCA any record that forms the basis for this report;
  • To challenge the accuracy and completeness of any information contained in the report;
  • To be informed whether CGAA or organizations associated with CGAA have denied your application because of the BCA's response and not to be required to pay the cost of the background check.

Your having provided this information to complete registration electronically binds you to the terms specified with the same force and commitment as if you had signed the document manually.  By completing the form, you acknowledge that you have read and understand the potential reasons for disqualification.
